What happens if an operative gives an incorrect additional question answer?

If you have additional questions and you have set questions to alert if the operative does not give the answer you require, this will show on the operative's log.

• For example. if you ask: Have you read and understood your RAMS? If the operative answers NO and you have set an alert, then this will show on the sites page as a red dot at the end of the line on this page

• From here you can then see the operative's log and the question they have got 'wrong':

To set an alert on the additional questions:

• Add your question
• Choose if this is to be Mandatory or not by checking the box under 'Is this question Mandatory'
• Select Yes or NO answer
• Choose the answer from the Flagged Question  - e.g. if you want the answer to  be YES, set the alert to show if they answer NO
